Detection of left atrial appendage thrombus by cardiac computed tomography: a word of caution.

چکیده
In this issue of iJACC, Martinez et al. (1) report their experience with cardiac computed tomography (CCT) to detect left atrial appendage thrombi in patients with atrial fibrillation undergoing pulmonary vein radiofrequency ablation. Atrial fibrillation is one of the most common cardiovascular disorders, carrying significant morbidity, including cardio-embolic stroke and heart failure. Slow flow in the left atrial appendage, also called spontaneous echo contrast, as well as left atrial thrombi have been associated with an increased risk of embolic stroke in patients evaluated by transesophageal echocardiography (TEE) (2). Evaluation by TEE is a safe, clinically accepted strategy for the assessment of atrial thrombus before cardioversion in patients with atrial fibrillation who have not received prolonged anticoagulation (3).

Severe Complications of Left Atrial Appendage Thrombi: a case report
Clinical significance of the left atrial appendage (LAA) is well-known for a cardiologist, but it's not so familiar to a radiologist. Patients with frequent dysrhythmia, especially with atrial fibrillation, often result in thrombus formation and subsequent cardioembolic events. This might be able to contribute to strokes, peripheral arteries occlusive disease (PAOD), or even death. We present a...

This study was conducted in 46 patients with cardiac thrombi, 15 patients with atrial myxomas, and 32 patients with other cardiac or paracardiac tumors. Diagnoses were subsequently proven by surgery, autopsy, computed tomography, magnetic resonance imaging, or angiography in all patients. All patients underwent precordial and transesophageal two-dimensional echocardiography to assess the variou...

PURPOSE To assess the diagnostic performance of 64-section cardiac computed tomographic (CT) angiography for detection of left atrial appendage (LAA) thrombi in stroke patients by using transesophageal echocardiography (TEE) as the reference standard. MATERIALS AND METHODS This study was approved by the institutional review board. Records were reviewed from 101 consecutive patients who had ex...
متن کاملComputed tomography for detecting left atrial thrombus: a meta-analysis
INTRODUCTION Thromboembolism, usually originating from the left atrium (LA) and left atrial appendage (LAA), is a major complication of atrial fibrillation and may result in transient ischemic attack and stroke. Computed tomography (CT) is a noninvasive test for detection of LA and LAA thrombus. We sought to conduct a meta-analysis to evaluate the accuracy of CT in detecting LA/LAA thrombus. ...
